Note History

The note history is helpful for recovering parts of your note that may have been edited, or for compliance/auditing purposes!

Updated today

A revision of your note is automatically created anytime the note is created, saved as a draft, moved, copied, transferred, or deleted.

This means you’ll always have a fallback option if that little trashcan button ever gets clicked by mistake 🙃 (we totally get it!).

You can view your note history in two ways:

  1. On a specific note – see the revision history for one particular note.

  2. For all notes – view all note revisions across your account in one place.


Note History for a Specific Note

You can view past revisions of a specific note in the web app. To do so, follow these steps:

⚠️ A new revision is only created when a note is saved as a draft. Autosaves do not create new revision updates.

  1. Open the note. Go to app.scribenote.com and open the note you want to view.

  2. Click the “... More” button. This button is located at the top-right corner of the note.

  3. Select “Note History" from the dropdown. This will take you to the Past Note Revisions page.

  4. View the list of revisions. You’ll see a list of .txt files, each representing a past revision of your note.

  5. Open a revision file. Click a file to open it. It should appear in a new browser tab (make sure pop-ups are allowed).

  6. Review the changes. In the text file:

    • Additions are marked with a +

    • Deletions are marked with a -

Note History for all Notes

You can view the revision history for all of your notes through your account settings. Follow these steps:

⚠️ If a note has been deleted, its recording cannot be recovered. Only the text revision will be available.

  1. Open your Account Settings. Go to your profile settings and select Account.

  2. Scroll to the Revisions section. Under Account settings, find the Revisions section.

  3. Click “See All Patient / Note Revisions.” This will open a list of .txt files showing all note revisions in your account.

  4. Find the note you’re looking for

    • Each file represents a revision event.

    • Deleted notes can be identified by filenames ending in note_delete_#.txt.

  5. Open a revision file. Click a file to open it. It will appear in a new browser tab (make sure pop-ups are allowed).

  6. Review the changes. Inside each text file:

    • Additions are marked with a +

    • Deletions are marked with a -
