Adaptive Templates

Customize Your templates!

Adaptive Templates are a Scribenote Pro feature that make note-taking easier by using AI to format your medical records just the way you like. You can either upload a blank note as a template or create your own from scratch.

You can also explore our Custom Template Library to save time and get inspired. These ready made templates can be copied directly into your account, helping you streamline workflows and keep records consistent across every patient.


Creating your Adaptive Template

⚠️ This feature is only available on your browser through the web app.

  1. Open Adaptive Templates. Click your name in the top right corner of your web app dashboard. From the dropdown menu, select Adaptive Templates.

  2. Click + New Adaptive Template.

  3. Name Your Template and Add a Description. In the Name field, enter a title for your template. Choose a name that will make it easy to identify later. In the Description field, write a brief description to help you quickly understand the purpose of the template.

  4. Choose Your Template Type. SOAP or Freeform note format.

    • SOAP Note: A structured format similar to the Scribenote Medical Record template. It is divided into Subjective, Objective, Assessment, and Plan sections that you can customize.

    • Freeform Note: An unstructured format with a single text box. This option allows you to create your own layout.

    • Need inspiration for your note? Select one of the preset options to have your template automatically populated. You can edit the preset by clicking into the text box and adding or removing lines.

  5. Choose How to Build Your Template. Automatic or From Scratch.

    • Automatic: Upload or drag and drop a PDF of an existing template and Scribenote will build it for you. If your template includes tables, Scribenote will not be able to replicate the exact format. All information from the template will still be captured though.

    • From Scratch: Manually type and create your template.

  6. Set Your Custom Defaults. To set your Custom Defaults, simply type them directly into your template where you would like them to appear.

  7. Fill Out Your Preferences. Preferences allow you to control the style and formatting of your notes. For more information on writing preferences, refer to the Template Preference Guide.

    Click on the examples provided for inspiration.

  8. Preview Your Template. Click Preview Changes to review your template and make any necessary updates.

  9. Save Your Template. When your template is ready, click Save at the bottom of the page. Your templates will be listed and organized on the Adaptive Templates page.

  10. Optional Template Actions. After saving your template, you can manage it from the Adaptive Templates page if needed.

    • Duplicate a Template: Create a copy of an existing template to reuse its structure without starting over.

    • Delete a Template: Remove templates you no longer need to keep your list organized.

    These actions are optional and are not required to create or use templates.

  11. Favourite Your Templates. To add a template to your Favourites, click the ⭐ icon in the top right corner of the template. Favourited templates will appear at the top of your list for easy access.


Recording with your Adaptive Template on the Web App

To use your new template, open the drop down menu on your main dashboard page. Select See All Templates to view your full list. Any templates you have favourited will appear at the top. Once you choose the template you want to use, click the purple record button to get started.

Recording with your Adaptive Template on the Mobile App

You can only change the template you are using once you begin recording on the mobile app. Start by clicking the purple record button at the bottom of the screen. When you begin recording, click the change arrows next to the template at the top of the screen. You will then see a list of all your available templates to choose from, and any templates you have favourited will appear at the top.


Sharing your Adaptive Template on the Web App

Once you have created your template, you can share it with other Scribenote Pro users. Start by saving the template to your account. After it is saved, select Share Template in the top right corner of the Adaptive Template page.

By default, your template will be restricted. You can choose to share it with your team, by email, or by link.

  1. Sharing with your team. This option allows all users on your team to view and use your template.

  2. Sharing via email. This option sends an email to the person you want to share the template with. The email includes a link that opens a view of the template. From there, they can select Copy to my Templates to save a copy to their own account and make any adjustments they need.

  3. Sharing via link. This option lets you make the link public. You can share the link with anyone. If they have a Scribenote Pro account, they can select Copy to my Templates to save a copy to their account.


Sharing your Adaptive Template on the Mobile App

Once you have created and saved your template on the web app, you can share it with other Scribenote Pro users. Open the mobile app and tap your profile image in the top right corner. Select View Adaptive Templates in the Templates section. You will see a share icon next to each adaptive template. Tap this icon for the template you want to share.

Setting Team Template Defaults

With Teams Mode, Team Admins can set Team Template Defaults. This ensures consistent records are produced across the entire clinic.To set this up, follow these steps on the web app.

  1. Open the Adaptive Template you'd like to set as a Template Default, then select Share Template -> Share with Team.

  2. If you're on one team: return to your Adaptive Template, then select Set as Team Default. It will now appear as a Default Template for your entire team!

  3. If you're on multiple teams: select Update Team Default Settings, then select the team(s) you'd like to set the Template Default to. It will now appear as a Default Template for the teams you've selected!
