Scribenote offers a variety of built-in template types to help you generate notes tailored to different workflows and appointment types.
All of the templates listed below are default templates available on the free plan.
If you’re looking to customize templates to better fit your clinic’s needs, you can upgrade to Scribenote Pro and use Adaptive Templates to create your own.

Medical Record
Medical Record
The Medical Record template is the most commonly used template in Scribenote.
It allows you to record your appointment and automatically generate a complete, fully formatted SOAP note.
Use this template when documenting standard appointments where you want a structured clinical note without manual typing.
This is a great default option if you are unsure which template to use.
Dental Record
Dental Record
The Dental Record template is designed for recording dental exams and automatically generating detailed notes with an interactive dental chart.
This template is ideal when you are probing teeth and want your findings mapped clearly to each tooth without manual charting.
Speak your findings naturally as you examine each tooth. For example:
“102 is mildly mobile, with mild tartar and gingivitis.”
“406 through 410 are normal.”
“The distal root of 206 and 207 is resorbing. 208 the palatal root is loose with a grade five pocket and pus present.”
Scribenote will determine species based on the teeth referenced, so you do not need to specify canine or feline.
You will get exam notes linked to each tooth, along with an interactive dental chart that allows you to review findings by hovering over individual teeth. Editing the note will automatically update the dental chart. After making changes, click Save Draft and refresh the page to see the updates reflected.
You can download the record as a PDF or send it as an email export. The chart will retain its formatting and colours.

Abd. Ultrasound

The Abdominal Ultrasound template is designed to generate a structured, systematic review of organ findings during or after completing an ultrasound scan.
Describe your findings as you move through each organ. Your recording will be processed into a clear, organized report that follows a consistent review format. This helps ensure nothing is missed and makes the note easy to read and share.
Multi-Pet SOAP
Multi-Pet SOAP
The Multi-Pet template is designed for appointments involving 2 to 3 pets during the same visit.
Use this template when examining multiple patients to generate separate, structured SOAP notes for each pet.
⚠️ Be sure to clearly state each pet’s name when describing findings, especially when switching between patients. This helps Scribenote accurately assign details to the correct record.
Pack Mode SOAP
Pack Mode SOAP
The Pack Mode template is designed for recording appointments involving larger groups of animals, typically 4 or more.
Use this template for litters, herds, flocks, colonies, or other group exams where multiple animals are assessed together.
Your recording will be processed into a structured SOAP note that captures the overall findings for the group.
This template is best suited for group-level documentation rather than individual patient details.

Dictation
The Dictation template generates a word for word transcript of your recording in a single text box.
Use this template when you need a full transcript instead of a structured medical note, such as for documentation, reports, or personal reference.
Your recording is captured exactly as spoken, without formatting or summarization.
You can also access a transcript from any note type after recording if needed.
Recap
Recap
The Recap template generates a concise summary of your recorded conversation in a single text box.
Use this template for meetings, general discussions, or any recording where you want key points summarized rather than fully transcribed or structured as a medical note.
Your recording will be condensed into a clear overview, making it easy to review and share.
Callback
Callback
The Callback template is designed for recording follow up calls with clients and generating a clear summary of the conversation.
Use this template when speaking with a client over the phone to create a concise overview of the discussion rather than a full transcript or SOAP note.
Start your recording, then begin your call. Scribenote will capture your side of the conversation and generate a summary in a single text box.
Scribenote will only capture your side of the call unless you place your phone on speaker so both voices are recorded.
